Singapore, 16 December 2019 - Embracing an innovative culture in Farrer Park Hospital (FPH), our staff initiated the #TheatreCapChallenge to enhance patient safety.

Originating from the U.S, the #TheatreCapChallenge aids in reducing communication error and staff misidentification by the simple move of labelling one’s name and role on the operating theatre cap. This implementation improves communication and familiarity amongst multi-disciplinary team through better name retention and teamwork in the operating theatres.

Johns Hopkins Medicine research revealed that preventable medical errors are identified to be the third leading cause of death in the U.S., resulting in over 250,000 deaths yearly from medical oversights. Although the World Health Organisation’s surgical safety checklist requires operating theatres to abide that all staff introduce themselves prior to surgery, there are still room for human error in identification during a strenuous operation.

According to Qual Saf Health Care research, communication failures occur in nearly 30% of operating team exchanges in theatre and a third of these have immediate effects such as causing inefficiency and tension.

The early adoption of this fresh yet practical #TheatreCapChallenge in FPH will contribute to strengthened trust, work engagement and clinical improvement within teams while ensuring patient safety.
