□ Passport: Ensure passport validity of a minimum of 6 months and above for international travel.
□ Visa: Please check with your respective embassy should you need to apply for an entry visa into Singapore. Visa applications can take up to 6 weeks to prepare, so plan ahead.
□ Medical Reports: Bring a hard copy of all medical reports that you have provided to specialist clinics and any new reports you have gathered since then.
□ Diagnostic Imaging Results: Bring the films and/or CD ROMs (DICOM compatible) of any diagnostic testing results relevant to your diagnosis (CT scan, MRI, PET scan, ultrasound, mammogram, X-ray, etc.).
□ Letter of Guarantee, if using your insurance
□ Local Currency (Singapore Dollars), or credit card for transactions.
