Atopic dermatitis (AD) is a chronic inflammatory skin condition influenced by genetic predisposition, immune dysregulation, and environmental triggers. Genotypic variations contribute to skin barrier dysfunction and heightened immune responses, predisposing individuals to early-onset and severe disease. Early intervention is critical, as it can modulate disease progression and reduce long-term complications. Pediatric patients, especially those with a family history of atopy, benefit from proactive skin care and targeted therapies to prevent flare-ups.

Recent advances have introduced novel biologic agents such as dupilumab, nemolizumab, and lebrikizumab, which target specific cytokines like IL-4, IL-13, and IL-31 involved in the Th2-mediated inflammatory pathway. These therapies offer improved disease control and reduced pruritus, especially in moderate-to-severe cases. However, a subset of patients remains difficult to treat due to disease heterogeneity, comorbidities, and resistance to conventional therapies. For these individuals, emerging strategies include bispecific biologics, JAK inhibitors, and personalized medicine guided by biomarkers and genotypic profiling.

Despite therapeutic progress, achieving minimal disease activity in refractory patients remains a challenge. Ongoing research into immune pathways and predictive diagnostics aims to refine treatment algorithms and enhance outcomes for all AD patients.
