
Esophageal cancer begins in the oesophagus — the long, muscular tube that carries chewed food from the throat down to the stomach. It most often starts in the cells lining the inside of the oesophagus, and can develop anywhere along its length, though certain sections are more commonly affected depending on the type of cancer involved.
Esophageal cancer is less common than some other cancers, but it carries a significant impact because it frequently causes few or no symptoms in its early stages, meaning it is often diagnosed only once the disease is more advanced. This is precisely why understanding the early warning symptoms — and not dismissing them — matters so much: outcomes for esophageal cancer diagnosed and treated early are considerably better than for disease found later.
The Two Main Types of Esophageal Cancer
Esophageal cancer is not a single disease, it falls into two main types, which differ in where they typically occur, their risk factors, and their global patterns.
- Squamous Cell Carcinoma: Arises from the flat cells lining the oesophagus, most often affecting the upper and middle portions. Strongly linked to tobacco and alcohol use, and historically the more common type in much of Asia, including Singapore, though its incidence has been declining in recent decades.
- Adenocarcinoma: Arises from glandular cells, most often in the lower oesophagus near the junction with the stomach. Closely linked to chronic acid reflux and Barrett's oesophagus, and has been rising in incidence in many Western countries alongside increasing rates of obesity and reflux disease, with a similar trend increasingly observed in parts of Asia.
Symptoms of Esophageal Cancer
Esophageal cancer often causes no symptoms in its earliest stages, which is one of the central challenges of this disease. Symptoms typically emerge as the tumour grows large enough to affect the passage of food or invade nearby structures.
- Difficulty swallowing (dysphagia) is often the most characteristic symptom, typically progressing gradually from difficulty with solid foods to difficulty with softer foods and eventually liquids
- Unexplained weight loss
- Chest pain, pressure, or burning, particularly with swallowing
- Worsening or persistent heartburn or indigestion
- A cough that doesn't resolve, or hoarseness
- Vomiting, or bringing food back up shortly after eating
- Vomiting blood, or black, tarry stools — signs of bleeding that require urgent assessment
- Fatigue, often related to weight loss or underlying anaemia from slow blood loss
Risk Factors for Esophageal Cancer
Risk factors differ somewhat between the two main types, though several overlap:
- Tobacco use: A major risk factor for squamous cell carcinoma in particular
- Alcohol consumption, especially heavy or long-term use, and particularly in combination with smoking
- Gastroesophageal reflux disease (GERD) and Barrett's oesophagus: The primary risk pathway for adenocarcinoma
- Obesity: Associated with increased reflux and a higher risk of adenocarcinoma
- Regularly drinking very hot beverages, which can cause chronic irritation to the lining of the esophagus)
- A diet low in fruits and vegetables
- Older age: Most cases occur in people over 50
- Male sex: Esophageal cancer is more common in men than women
- A history of other head and neck cancers, which shares some risk factors with squamous cell carcinoma
How Esophageal Cancer is Diagnosed and Staged
Upper Endoscopy with Biopsy
The primary diagnostic test. A thin, flexible camera is passed down the oesophagus to directly visualise any abnormal areas, with tissue samples (biopsies) taken for laboratory analysis to confirm a cancer diagnosis and identify its type.
Endoscopic Ultrasound
Combines endoscopy with ultrasound imaging to assess how deeply a tumour has grown into the wall of the oesophagus and whether nearby lymph nodes appear to be involved — key information for staging and treatment planning.
CT Scan
Provides detailed cross-sectional imaging of the chest and abdomen to assess the extent of the tumour and check for spread to lymph nodes or distant organs.
PET Scan
Highlights areas of increased metabolic activity throughout the body, helping to detect spread that may not be visible on CT alone, and often used alongside CT for complete staging.
Staging Laparoscopy
A minimally invasive surgical procedure occasionally used, particularly for tumours near the stomach, to directly inspect the abdominal cavity for small-volume spread that imaging may miss before finalising a treatment plan.
Staging describes how far the cancer has spread — from early-stage disease confined to the lining of the oesophagus, through locally advanced disease involving deeper layers or nearby lymph nodes, to metastatic disease that has spread to distant organs. Stage is the single most important factor in determining which treatment approach, or combination of approaches, is most appropriate.
Reducing Your Risk of Esophageal Cancer
There is no guaranteed way to prevent esophageal cancer, but several steps can meaningfully lower your risk:
- Avoid tobacco in all its forms. If you smoke, speak to your doctor about support and aids to help you quit
- Limit alcohol intake, particularly heavy or long-term use, and especially in combination with smoking
- Manage chronic reflux (GERD) actively rather than simply tolerating it — persistent, poorly controlled reflux is the main driver of the adenocarcinoma risk pathway
- If you have been diagnosed with Barrett's oesophagus, attend your recommended surveillance endoscopies, since this is specifically designed to catch any progression early
- Maintain a healthy weight through regular exercise and a balanced diet
- Eat a diet rich in fruits and vegetables, and obtain vitamins and nutrients from food sources rather than high-dose supplements
- Avoid regularly drinking very hot beverages
- Do not dismiss new or worsening difficulty swallowing as something to simply work around, seek a healthcare professional's medical assessment
