Endoscopic retrograde cholangiopancreatography or ERCP was first developed in 1968 by an Obstetrician. It was used widely from the 1970’s onwards and in 1973, the first sphincterotomies were performed. Since then, ERCP has evolved from a diagnostic to a therapeutic tool.
ERCP is used mainly to treat biliary and/or pancreatic obstruction resulting from choledocholithiasis, acute cholangitis, malignant biliary obstruction, post-surgical biliary obstruction, pancreatitis, primary sclerosing cholangitis and sphincter of Oddi dysfunction.
The first percutaneous choledochoscope (biliary endoscope) was described in 1941 but it was not until the 1970’s when the first peroral cholangioscopes (mother-baby scopes) were introduced. These required 2 operators, were difficult to use and prone to malfunction.
In 2007, Boston Scientific developed the first single operator cholangioscope – the SpyGlass™ system which overcame the limitations of traditional cholangioscopes.
Finally in 2015, the next generation SpyGlass™ system, the DS Direct Visualization system was introduced. The SpyScope™ is also equipped with a working channel which allows passage of laser or EHL fibers through to blast difficult biliary/pancreatic stones. More instruments are available for use currently including biopsy forceps, snares and baskets.
