High Intensity Focused Ultrasound (HIFU)

Farrer Park Hospital is the first in Southeast Asia to perform the HIFU treatment for uterine fibroid.

High Intensity Focused Ultrasound (HIFU) or Focus Ultrasound Surgery (FUS) is a new technology for the treatment of Uterine Fibroids, Adenomyoma, 
Adenomyosis and certain cancers.
It is a non-invasive surgery that uses focused ultrasound to generate highly localized heat to treat tumors either benign or malignant.

Why HIFU?
  • Computerized, non-invasive surgical procedure, which does not require surgical incision
  • No loss of blood and no need for blood transfusion
  • Focused ultrasound uses an acoustic lens to concentrate multiple intersecting beams of ultrasound on a target
  • Nearly no damage to surrounding healthy tissues  
  • May keep uterine function 
  • Short treatment time 
  • Short recovery period 

What are Uterine Fibroids?
Uterine fibroids are non-cancerous tumors that grow along or within 
the walls of the uterus. 

They are primarily made up of smooth muscle cells along with small amounts of other tissues. They can be of various sizes. Some fibroids are microscopic whereas others may be more than 10cm across. Fibroids can be found alone or in clusters.

The exact cause of Uterine Fibroids is still unknown but it is believed that genes, hormones and endocrine system as well as environmental factors might contribute to Uterine Fibroids.

Symptoms of Uterine Fibroids
Many women who have fibroids do not have any symptoms. However, symptoms can present itself based on the locations, size and number of fibroids. The most common symptoms of uterine fibroids include:

  • Heavy menstrual bleeding
  • Menstrual periods lasting more than a week
  • Pelvic pressure or pain
  • Frequent urination
  • Difficulty emptying the bladder
  • Backache or leg pains

Prevalence of Uterine Fibroids
Uterine Fibroids are the most common type of gynecologic​ benign tumors occurring in 25% to 50% of women during their reproductive years. 

The conventional treatment for symptomatic women is hysterectomy or myomectomy.

What to Expect

Before Treatment
  • No sex 3 days before treatment and make sure IUD, if any, is removed before treatment.
  • Full shave of pubic area is required.
  • Bowel preparation: enema.

Diet preparation: Light diet low in vegetables and beans 3 days before treatment; liquid diet 1 day before treat-ment and no eating 8 hours before treatment.

After Treatment 
  • Lie prone for at least 45 min after treatment.
  • Liquid food 2 hours after treatment is allowed and normal diet can be resumed 24 hours after treatment if there is no abdominal pain, abdominal distension, local tenderness, fever or loss of appetite.
  • Keep genitals clean after treatment.
  • No sex after treatment; it can be resumed after one menstrual cycle.
  • Temporary contraception is advised for a few months.
  • Placement of IUD should be after 3 normal menstrual cycles. 
  • Avoid intrauterine operation during this period.
  • Post operative follow up with ultrasound or MRI scan is advised.

Possible Postoperative Reactions
  • Distending pain in treatment area, lower abdomen, sacrum area and legs; Temporary weakness in the legs. 
  • Redness of skin in treatment area.
  • Dizziness due to medication, low blood glucose level etc.  Have a good rest and a nutritious diet. 
  • The first (and even the second) menstruation after treatment may increase or lengthen.
  • It usually takes 3 months for symptoms to improve significantly.
  • Patients with submucosal fibroids may have secretions lasting about one week after treatment. Keep the genitals clean.
